第三节 门静脉系统血栓分型
研究发现,门静脉系统血栓形成相关的危险因素包括肿瘤、肠道炎症、腹部手术史、感染等,其中肝硬化是形成PVT的重要危险因素之一[1]。肝硬化合并PVT的发生率为5%~36%,其中以肝功能较差的肝硬化患者更为显著[2]。门静脉系统血栓形成的患病率在整体人群中仅为1.1%[3],在肝硬化患者中为10%~25%[4],在肝移植手术患者中为2.1%~26%[5]。另外,门静脉系统血栓是脾切除术后严重的并发症之一,尤其是肝硬化门静脉高压症患者。尽管脾切除术后的前期临床表现往往并不严重,并且缺乏特异性,但后期仍可能导致缺血性肠坏死、消化道出血、肝衰竭等严重后果。国内外研究报道称,脾切除术后PVT发生率为0.6%~15.8%[6]。肝硬化PVT患者可反复出现上消化道大出血和顽固性腹水,严重威胁患者生命[7]。
门静脉系统血栓指门静脉主干及其分支的血栓形成,其评估主要包括血栓的程度(包括部分血栓、完全血栓和纤维条索形成)[8]、血栓的分期(包括急性血栓、慢性血栓和门静脉海绵样变性)[9]和血栓的范围(包括门静脉主干、门静脉左右支、脾静脉或肠系膜上静脉)。我们将从以上3个方面结合既往文献报道对门静脉系统血栓分型进行阐述。
Qi等[10]依据腹部增强CT的扫描结果对门脉血栓进行如下分级:部分阻塞是指血栓阻塞血管面积大于1/2;完全血栓是指血栓面积几乎占满血管管腔;门脉闭塞是指门脉完全消失或海绵样变性。Luca等[11]、Bauer等[12]将门静脉系统血栓按血栓阻塞血管的程度分为4个等级:Ⅰ级,阻塞体积占管腔的比例为1%~25%;Ⅱ级,阻塞体积占管腔的比例为26%~50%;Ⅲ级,阻塞体积占管腔的比例为51%~75%;Ⅳ级,阻塞体积占管腔的比例为76%~100%。此种分级方法也是依据影像学,如螺旋CT或磁共振血管造影(MRA),对血栓进行测量的,分级标准更为详细,多位学者应用此标准,更容易达成共识。
对于急性和慢性门脉血栓的定义一直存在争议,一般是根据急性腹痛的起病时间来区分,不同的学者对时间有不同的定义,包括 14[13]、40[14]、60[15-17]、90[18]天,门脉海绵样变性肯定是慢性血栓的标志。Qi等[9]进行了如下的定义,急性门脉血栓是指没有侧支循环形成,且符合以下任意一个标准:①由门脉血栓导致的14天内的急性腹痛或肠缺血或肠坏死,且既往没有慢性血栓病史;②在腹部CT平扫中可见门静脉内高密度影。慢性血栓是具有以下3个特征之一:①在腹部增强CT中可见门静脉内密度减低影;②原门脉主干消失或出现门脉海绵样变性;③门脉海绵样变性。Han等[8]认为慢性血栓不等同于门脉海绵样变性及肝功能失代偿,如要对急慢性门脉血栓的区分达成一致,必须对以下概念达成一致,其中包括急性腹痛的起病时间窗口期、急性血栓与门脉海绵样变性的时间间隔、急慢性血栓的定义、无症状患者的慢性血栓。Senzolo等[19]将发现门脉血栓的时间进行划分:①小于6个月;②6~12个月;③大于12个月。其目的是指导治疗,他认为小于6个月的门脉血栓抗凝治疗效果好。
Luca等[10]对PVT进行了如下分型:按血栓的部位分为门脉主干血栓、肠系膜上静脉血栓、脾静脉血栓、门脉主干+肠系膜上静脉血栓、门脉主干+肠系膜上静脉血栓+脾静脉血栓。肝移植术后极易发生门脉系统血栓,很多学者针对此类情况对PVT进行了不同分型。Jamieson等[20]进行了如下分型:Ⅰ型,血栓局限在门静脉或门静脉与肠系膜上静脉或脾静脉的交汇口,血栓为完全性或部分性;Ⅱ型,血栓由门静脉延伸到肠系膜上静脉近端,但肠系膜上静脉血流通畅;Ⅲ型,血栓局限在门静脉系统,但有大量侧支循环血管形成;Ⅳ型,血栓广泛分布在门静脉系统,仅有少量侧支循环血管形成。Nonami等[21]将PVT分为:Ⅰ型,血栓局限在肝内门静脉;Ⅱ型,血栓在门静脉左支、右支或左右支分叉处;Ⅲ型,部分血栓局限在门静脉主干;Ⅳ型,整个血栓在门静脉主干。Stieber等[22]将PVT进行分型:Ⅰ级,门静脉有部分血栓;Ⅱ级,门静脉完全血栓合并或不合并脾静脉血栓;Ⅲ级,包括肠系膜上静脉在内的广泛血栓;Ⅳ级,血管内广泛血栓,血管不能显示。Yerdel等[23]对门静脉血栓进行如下4个分级:Ⅰ级,小于门静脉管腔50%,局限,未向肠系膜上静脉延伸;Ⅱ级,门静脉阻塞程度在50%~100%,有或没有向肠系膜上静脉延伸;Ⅲ级,门静脉及近端肠系膜上静脉完全阻塞,而远端肠系膜上静脉尚通畅;Ⅳ级,门静脉、近端及远端肠系膜上静脉完全阻塞。赵洪伟等[24]对门静脉血栓进行如下4个分型:Ⅰ型,单纯门静脉主干血栓形成;Ⅱ型,门静脉主干合并所属分支血栓形成,Ⅱa型门静脉主干合并肝内分支血栓形成,Ⅱb型门静脉主干合并脾静脉血栓形成,Ⅱc型门静脉主干合并肠系膜上静脉血栓形成;Ⅲ型,单纯门静脉所属分支血栓形成,Ⅲa型单纯门静脉肝内分支血栓形成,Ⅲb型单纯脾静脉血栓形成,Ⅲc型单纯肠系膜上静脉血栓形成;Ⅳ型,混合血栓形成(包括门静脉海绵样变性,但肝内分支正常;门静脉系统完全海绵样变性除外)。
首都医科大学附属北京世纪坛医院、中国人民解放军第三〇二医院等医院的编委们对3052例(其中1104例进行了介入治疗)门静脉系统血栓患者的解剖发生部位进行了总结,并进行了解剖分型,结果如下:Ⅰ型,门静脉主干血栓813例,占26.6%(图1-3-1~图1-3-3);Ⅱ型,门静脉分支血栓319例,占10.5%(图1-3-4~图1-3-6);Ⅲ型,门静脉属支血栓231例,占 7.6%(图1-3-7~图1-3-12);Ⅳ型,联合血栓 1464例,占 48.0%(图1-3-13~图1-3-42);Ⅴ型,海绵样变性145例,占4.8%(图1-3-43~图1-3-45);Ⅵ型,肝移植后门静脉血栓13例,占0.4%(图1-3-46和图1-3-47);Ⅶ型,TIPS后门静脉血栓 56例,占 1.8%(图1-3-48~图1-3-50)。每一种类型分为两个亚型:a表示原血管血流通畅,b表示原血管血流不通。
